The participant agrees that:

• They may be covered under Surfrider Foundation Limited’s Public Liability Insurance for the undersigned only during the time of the activity and on the date shown; however, Surfrider cannot make any guarantees in relation to this coverage.
• Surfrider Foundation Limited will not be liable for: any acts or omissions, negligence or fault of any person participating in the activity; any loss or damage to property.
• The participant warrants that all of the details below are true and correct.
• If your photo is taken, it may be reproduced in any form and distributed, published or broadcast by any medium, including the internet / social media, television, cinema and multimedia.
• IMPORTANT! Parents and Guardians must accompany and sign on behalf of any children under 18 years of age.
